Do higher incomes mean higher Medicare costs?

Yes. Cross certain income thresholds and a surcharge called IRMAA (Income-Related Monthly Adjustment Amount) gets added to your Part B and Part D premiums.

The number that matters is your Modified Adjusted Gross Income from two years back, and couples get their own set of brackets.

Other costs to consider

  • Medigap premiums: a Supplement plan adds its own monthly premium, which moves with your ZIP code, your age, and the plan letter you choose.
  • Late enrollment penalties: delay Part B or Part D past your window and the penalty attaches to your premium permanently.
  • Dental, vision, hearing: Original Medicare pays nothing here; budget for standalone coverage or an Advantage plan that includes it.
